Item-typ:Veröffentlichung, Development of a patient assessment to meet the needs of patients suffering from advanced non-oncological diseases – the KOPAL studyBackground To develop an aide memoire for patients diagnosed with advanced non-oncological chronic diseases, the KOPAL conversation guide, to foster interprofessional counselling between GPs and specialist palliative home care teams. Methods As part of the multi-centre, two-arm, cluster randomised controlled KOPAL study, the draft of the conversation guide was discussed and consented in three focus groups with 4 patients, 7 health care providers, and 5 stakeholders. Results The final version of the KOPAL conversation guide contains eight key topics: living with the illness, physical situation, emotional situation, personal situation, social situation, information and communication, control and autonomy, emergency management. Each topic refers to different general points, which are listed on top of the respective thematic section. The conversation should start by addressing the patient’s today’s well-being and end by asking for the patient’s main topic based on the discussed issues. Conclusions The KOPAL conversation guide is a broad evaluation and communication tool that covers potential palliative care (PC) needs of non-oncological patients and provides a basis for interprofessional case planning and counselling. Applying the KOPAL conversation guide may help to bridge the communicative gap between general and specialist PC professionals and between professionals and patients.Wissenschaftlicher Artikel88 - Some of the metrics are blocked by yourconsent settings

Item-typ:Veröffentlichung, Behandlung im Voraus Planen – ein «Advance Care Planning»-Konzept für DeutschlandDas internationale Konzept von «Advance Care Planning» (ACP) steht für einen systematischen und qualifizierten Gesprächsprozess zwischen Personen, die über mögliche Krankheitsszenarien in Anwesenheit ihrer Angehörigen und Therapeuten sprechen möchten und ihren Wunsch zu medizinischen, pflegerischen, psychosozialen und seelsorglichen Versorgungen und Behandlungen äussern wollen. In Deutschland wurde im Sozialgesetzbuch (SGB) V der § 132 g zur Gesundheitlichen Versorgungsplanung für die letzte Lebensphase als Versicherungsleistung der Gesetzlichen Krankenkassen (GKV) für bestimmte Patientengruppen festgesetzt, was das ACP-Konzept widerspiegelt. Ein ACP-Konzept, das auf die Strukturen des Gesundheitssystems in Deutschland ausgerichtet ist, ist als «Behandlung im Voraus Planen» (BVP) bundesweit bekannt geworden. Im Rahmen der BVP-Gespräche werden nach dem Prinzip der partizipativen Entscheidungsfindung mit Bewohnern von Senioreneinrichtungen oder mit Menschen in der Eingliederungshilfe die individuellen Wünsche zu zukünftiger Behandlung und Versorgung erfasst und besprochen. In einem Dokument, wie einer Patientenverfügung, können diese Wünsche detailliert festgehalten werden. Diese Gespräche können hierfür speziell ausgebildete Personen aus den ärztlichen, aber auch nicht ärztlichen Berufsgruppen – wie Pflegefachkräfte, Sozialarbeiter, Heilerziehungspfleger, führen. VSED is controversial, and research is essential to inform this debate. The aim was to systematically synthesize empirical data on the attitudes, motives, and experiences related to VSED from the perspective of caregivers. Methods: This systematic mixed-methods review (PROSPERO CRD42022283743) included qualitative and quantitative research. The MEDLINE, CINAHL, PsycINFO, Google Scholar, and BELIT databases were searched for English and German articles published between 1 January 2013 and 12 November 2021. Studies examining experiences, attitudes, and knowledge regarding VSED were eligible. We analyzed the data inductively after quantitative data transformation. Quality and confidence were assessed using the Mixed Methods Appraisal Tool (MMAT) and GRADE-CERQual approach, respectively. Results: We identified 22 articles, including 16 studies. The participants were healthcare professionals and relatives, but not those who chose VSED. The motives for VSED are based on high symptom burdens and are closely related to self-determination. Most perceive VSED as a natural death and accept the patient’s decision. However, this acceptance also depends on other factors, such as patient characteristics. Most healthcare professionals would accompany patients during VSED, sometimes leading to advocacy. Relatives often play an active role in VSED and may experience distressing grief if they struggle with their support or the dying situation. The confidence in the review findings ranged from moderate to very low. Our findings on the challenging and positive experiences related to VSED and the resulting needs have been published elsewhere. Conclusions: VSED is a complex phenomenon affecting patients, nursing staff, physicians, and relatives.
